Registration Agreement

This agreement is made as of ______________________, 2007, between Janet Hall (Sponsor), whose address is 9809 Candelaria NE Ste. 1B, Albuquerque, NM 87112 and (Student), whose address is ____________________________________, Phone # is______________________, e-mail address is ________________________________.

1. The Course: The sponsor shall hire instructors to conduct class, and the student shall participate in The Professional Herbalist Certification Course to be conducted as follows:

A. Course Location: Albuquerque, NM

B. Course Times: 9:00 am – 5:30 pm

C. Course Dates: (Dates to be announced)

2. Course Fee: The student shall pay the Sponsor a course fee of $2,450.00, plus New Mexico State Tax (6.875%), totaling $2,618.44 on the following schedule:

A. Initial deposit in the amount of $1,309.22. This amount must be sumitted along with this registration form. (Non-refundable unless the Sponsor cancels the course).

B. The remaining balance of $1,309.22 is due 10 days prior to the first day of class. (Non-refundable unless Sponsor should cancel the course.)

3. Certificate: Upon the student’s satisfactory completion of the course and the Student’s payment to the Sponsor of entire course fee, the Instructor shall provide the student with a Professional Herbalist Course Completion Certificate.

4. Release and Hold Harmless: The Student hereby releases and holds harmless the Sponsor and each of the Sponsor’s agents, employees, trainers, and independent contractors from all liability for any physical, mental or emotional injury, or any loss or damage to personal property, which the Student may have suffered in the past or may suffer in the future arising out of or in relation to the course. The Student Understands that his/her application for registration in the course will not be accepted if the Student refused to be bound by the provisions of the paragraph. The Sponsor is not responsible for tutoring the Student. It is understood by the Student that he/she has the responsibility to arrange study groups with the other students as needed or obtain the guidance needed from the Instructor of the Class. The Student understands that course tuition does not include textbooks, and the required/recommended books are to be purchased separately as needed by the student. The beginning and ending dates of the course may vary slightly, and students will be given ample notice for adjustments to the schedule. The Student understands that the Instructors may have delayed flight times arriving in Albuquerque due to Airport schedules/circumstances beyond the Sponsors control and that the class may not always start/end according to schedule. The class location may vary, but will remain in the Albuquerque area. Student hereby releases and holds harmless the Sponsor and Instructors from all liability in these matters.
